Terumo Corporation (hereinafter referred to as "Terumo") may sometimes receive your personal information (hereinafter referred to as the "Information") for the purpose of providing you of valuable goods and services in the medical care field. In such case, Terumo will make efforts to properly treat and protect the Information according to the Personal Information Protection Policy (hereinafter referred to this "Policy") as below.

Please be noted that this Policy is intented for residents in Japan and based on the Japanese “Act on the Protection of Personal Information”. This Policy is an English translation and for reference purpose only. Please confirm the official Policy written in Japanese if necessary.

Personal Information Protection Policy

  1. Use of the Information
    If Terumo is provided with certain information by you, Terumo will clearly show the purpose of use of such information to you and will never use it for other purposes than the purpose of use shown to you without your consent.
  2. Providing the Information to third parties
    Terumo will not provide nor disclose the Information to any third party without your consent or required by laws and regulations.
    If Terumo discloses the Information for the purpose of use above to Terumo’s affiliates and/or outsourcing companies, Terumo will take reasonable measures, including concluding confidentiality agreement, to properly treat and protect the Information.
  3. Security of the Information
    Terumo takes security measures technically and organizationally, including placing a person in charge of management and/or control of the Information, to prevent loss, destruction, falsification, divulgence or leakage to outside Terumo of and unauthorized access to the Information.
  4. Disclosure, correction, deletion and suspended use of the Information
    If you intend to disclose, correct or delete all or any part of the Information or suspend the use thereof please contact the information in Section 6 below .Terumo will promptly follow your request within the reasonable extent.
  5. Revision of this Policy
    Terumo will review and revise this Policy from time to time to protect the Information, and will disclose the revision of this Policy, if any, on this website.

Purposes of Use of the Information and the personal data held by Terumo

1. Purposes of use of the Information of physicians, dentists, pharmacists, pharmacies, researchers and other healthcare providers:

  • To provide and collect information related to the proper use of pharmaceuticals and medical devices;
  • To provide and collect information related to quality, safety and/or effectiveness of pharmaceuticals and medical devices;
  • To investigate and research in the fields of medicine, pharmacology and medical devices;
  • To provide and collect medical care and health information and academic information;
  • To request for and carry out clinical trials, post-marketing surveillances and other matters;
  • To submit and/or make reports to the governmental authorities;
  • To deal with inquiries, document requests and other requirements; and
  • To authorize the access to Terumo’s website for healthcare providers, and to inform about the authorization.

2. Purposes of use of home care patients’ Information:

  • To provide and collect information related to the delivery and proper use of home-use medical devices and pharmaceuticals;
  • To provide and collect information related to quality and/or safety of home-use medical devices and pharmaceuticals; and
  • To communicate necessary matters to medical institutions and other entities and/or to provide information to them.

3. Purposes of use of the Information of online shopping users:

  • To deal with the affairs related to the delivery of the goods and invoice, collection of the price thereof and otherwise; and
  • To deal with inquiries and requests for materials and other requirements.

4. Purposes of use of Information of business partners:

  • To provide, collect and communicate information, distributions and other things related to business activities; and
  • To provide, collect and communicate information related to the materials, procurement and other matters.

5. Purposes of use of the Information of users of the Public Relations Office, call centers and any other customer service of Terumo:

  • To consider, investigate and deal with consultation, communications and otherwise;
  • To communicate and provide information to the manufacturers and sellers of products, healthcare providers and others; and
  • To make reports to the government authorities.
  • Terumo may sometimes record the phone call contents to keep accuracy and correctness of the details of consultation and communication by you.
  • Terumo may also call your phone number shown as caller’s ID to explain about the consultation, communication and other contacts.

6. Purposes of use of the Information of Terumo’s shareholders:

  • To perform the obligations in the Companies Act and to inform how to exercise the shareholders’ right and deal with the affairs related to the exercise thereof;
  • To send the Business Report and other distributions; and
  • To promote exercising the voting right and to inform the shareholders’ benefits.

7. Purposes of use of the Information of the applicants for employment:

  • To consider about and decide the employment;
  • To inform the prior procedures to enter Terumo; and
  • To deal with inquiries and requests for materials and other requirements.

8. Purposes of use of the Information of the current, resigned and retired employees, and their family members:

  • To manage the work, salary payment, personnel affairs, evaluation, development of ability, welfare, health and safety and other matters;
  • To communicate and provide to the labor union, health insurance societies, pension funds, subsidiaries and affiliates;
  • To submit and/or make reports to the governmental authorities; and
  • To make urgent contact.

9. Purposes of use of Information of the visitors and applicants for visit to Terumo’s facilities:

  • To instruct and treat the visitors in visiting and entering the facilities; and
  • To manage the inquiries, questionnaires and otherwise after visiting and leaving the facilities.

10. Purposes of use of the Information of the employees of the counterparts of Terumo’s contracts and agreements:

  • To efficiently perform the work; and
  • To communicate necessary matters and make inquiries to such counterparts.

Joint use of the personal data

1. Terumo’s subsidiaries and affiliate companies, health insurance society, holding company and pension fund:

  • Purpose of use
    • To properly manage the current, resigned and retired employees and their family members between and among the joint users.
  • Scope of joint users
    • Terumo’s subsidiaries and affiliate companies, health insurance society, holding company and pension fund.
  • Personal data items
    • The personnel management information, including but not limited to employee number, name, sex, date of birth, date of entry to Terumo, post, employment status and other matters, of the current, resigned and retired employees and their respective family members held by each of the joint users and any other data necessary to complete the purposes of use stipulated above.

2. Terumo’s labor union

  • Purposes of use
    • For Terumo to communicate the various internal matters and give notices, announcements and otherwise to union members, and for the joint users to give notices on the policies and make reports on the labor-management consultation (negotiation) and give information of the events to such members;
    • To create the basic data on the wage and working conditions of union members for the labor-management agreement (negotiation) between and among the joint users;
    • To take efficient and proper measures by way of the mutual cooperation between and among the joint users in a natural and/or man-made disaster and also in emergencies of Terumo and an individual member, including in case of physical and mental health problems and other conditions; and
    • To practice the employees’ benefits business presided by the joint users.

    • The personnel management information, including but not limited to employee number, name, sex, date of birth, date of entry to Terumo, post, employment status, personnel treatment data necessary for the labor-management, of the current, resigned and retired employees and their respective family members held by each of the joint users and any other data necessary to complete the purposes of use stipulated above.

  • Purpose of use
    • To deliver the goods sold by mediation and charge for and to collect or recall the goods, and to make various kinds of inquiries and requests for materials and after-sales services and other requirements;

  • Issues of the personal data
    • The data necessary, including the name, address, phone number, to complete the purposes of use above.

Terumo is a business entity subject to the organization below, an authorized organization of the personal information protection authorized by the Ministry of Health, Labour and Welfare.
